Working with Navexio Oncology Nurse Navigators
Process
The following is an overview of the steps involved in working with Navexio’s Oncology Nurse Navigators for virtual and in-person services.
In-Person Consultations

A Navexio Oncology Nurse Navigator will ensure effective preparation for the appointment by reviewing the Client’s medical records (if available).
-
A Navexio Oncology Nurse Navigator will schedule an initial consultation to learn about the patient’s cancer journey, medical history, and discuss their needs and challenges. This will set the course for Navexio to develop a personalized oncology navigation and consultation roadmap.

-
The Nurse Navigator will listen and ask appropriate questions to the the oncologist and members of the clinical care team and take appointment notes. The Nurse Navigator will identify and help mitigate barriers and roadblocks in the cancer journey.
-
The Nurse Navigator will create a summary report of the appointment and provide a tailored action plan, reminders, and next steps until the next appointment.
